The problem

Hospital discharge can happen fast. Then the family is left holding the medicine changes, the visits, the letters and the worry, often from miles away. Families juggle group chats, paper letters and appointments across several people and several homes.

What we built

  • A care circle. Family and friends in one place. Everyone can see who is visiting, who is collecting the prescription and what has been done. Family join free, and the person looked after never pays.
  • A daily "I'm OK". One big button, also on Apple Watch. If it has not been tapped by the time the family chooses, the circle gets follow ups in the order they set. It is a daily hello, not an emergency service.
  • Medicines. A shared list with reminders and a record of each dose, and who recorded it.
  • Letters in plain English. Scan an NHS, hospital, council or DWP letter and get a plain English summary, key dates and who to contact, for the family to check.
  • Help you might be entitled to. An entitlements guide covering Attendance Allowance, Carer's Allowance and more. Guidance only: DWP and councils decide.
  • Coming home from hospital. Questions to ask the ward, a home readiness list and a shared 30 day plan.

Privacy and safety by design

  • The person being cared for owns their information and chooses what to share, type by type, and with whom.
  • No adverts, no ad trackers, and information is never sold.
  • Everything can be exported or deleted at any time.
  • Coming Home is not a medical device and does not give medical advice. Every relevant screen says: "If you're worried about their health, contact their GP or NHS 111. In an emergency call 999."

Going to market

Alongside the app, we built the website at cominghomecare.app with a waitlist, a free Benefits finder, a library of practical guides for carers, and a go to market plan.

Where it is now

A friends beta is planned from mid October 2026 and a wider beta from mid November 2026. Launch is planned for February 2027.

What this means for clients

Building our own product means we have done every step ourselves: research, product design, the app, the website, pricing, privacy and launch planning. That is the experience we bring to client projects.

Why sometimes less is more

Fewer features, fewer messages, fewer steps. Why doing less, on purpose, often makes products, brands and services better for the people who use them.
